THE KAOS SHOW CHOIR REGISTRATION FORM

To register for our KAOS Show Choir please fill in the following form completely. After you complete the form, hit the "Submit" button and proceed to the deposit payment page to complete your application. At this page, you will be asked to pay a $50 deposit to complete your registration. Once we have assembled the various groups you will be notified by email. The balance of $400 will be due to be paid by no later then Sept 1st. We will send you a reminder email and you can pay the balance in our store or online.
